How to complete the Under One Roof registration form

In sections 1, 2, 4 and 5 of the registration form we ask you to provide evidence to support what you tell us about where you live, your housing need and so on. The following information is to help you to include the right information to us with your application. It is very important that you send as much information as possible. If you don’t we may be unable to process your application and have to send it back to you.

Sections that we need you to include additional information are…

1 About You

We need to know that you are who you say you are – you can prove this by sending us a photocopy of your driving license, passport detail or your birth certificate. We also need you to prove that you live at your address. You can do this by sending us a photocopy of a gas, water or electricity bill or a statement from a bank, credit card or storecard. If you don’t send in something to prove who you are or where you live we will send your form back to you asking for it.

2 Details of household members

If your children are amongst those in your household please provide a photocopy of your child benefit or tax credits letter which must include the name of each child on it. We can only consider your children as part of your application if you include this information.

3 Where you used to live

Please provide details of your last 6 years addresses including any periods where you did not have a fixed address or you were staying somewhere temporarily. If you currently hold or have previously held a tenancy please provide your landlords details as without these your form will be returned to you. It is very important that you provide all your previous landlords details. Failure to do so may mean that we can’t make you a member of the scheme until you provide this information.

4 Why do you want/need to move?

Any information that you can provide to support this section will help us to process your application more accurately and quickly. Information you might want to include are copies of: a letter from your employer, details of someone that you support or care for, any information that proves relationship breakdown, notification of a compulsory purchase order (CPO) from the local authority or an Environmental Health report if your situation includes poor living conditions. If you have been served with a Notice To Quit by your landlord, are in financial difficulties or are at risk of becoming homeless, please contact St Helens Council’s Housing Options and Advice Service for further advice and assistance on your circumstances.

Any evidence that backs up your situation will mean we can process your application quicker and also mean that you can start bidding for homes quicker too.
