How to complete the Under One Roof application form

When you complete an application form for Under One Roof you have to do the following to make sure we can process your application as quickly as possible:

  • Complete the application form in full and check that the information is correct.
  • Make sure that there are no gaps in your address history. If there are any gaps we won't be able to process your application.
  • Sign and date the application form at the bottom of Section 17. If there is a joint applicant, you will both need to sign the form.

We need you to provide evidence to support what you are telling us about who you are, where you live, your housing circumstances, your connections to St Helens and your previous addresses.

Online Applicants

You will need to complete the application form in full with all of the information that applies to you. You will need to tick a box at the end of Section 17 to confirm all the information you have given us is correct, otherwise you won't be able to submit your form to us. You will also need to provide us with an email address so that we can use this for corresponding with you in future.

All applicants

All applicants need to provide evidence to support what you are telling us about who you are, where you live, your housing circumstances, your connections to St Helens and your previous addresses

The information below will help you to include the right information when you send your application form back to us.

It is very important that you provide the correct information otherwise we won't be able to process your application. Please do not send any original documents, photocopies are acceptable.

We cannot accept any responsibility for the loss of any original documents sent to us in the post.

Section 1 - About You

We need to know that you are who you say you are. We need proof of identity for you and any joint applicant.

We need a photocopy of one of the following for you and any joint applicant:

  • Passport
  • Driving Licence
  • Birth Certificate
  • Benefit letter that includes your National Insurance Number

We also need proof of the current address that you and any joint applicant are living at. This must be dated within the last 6 months.

We need a photocopy of one of the following for you and any joint applicant:

  • Gas, water, electric or mobile phone bill
  • Credit or store card statement
  • Letter from your bank

If you don't send the correct documents, we won't be able to process your application.

Are you pregnant? You will need to provide proof of your pregnancy and due date (MATB1 form).

Section 2 - Details of household members

Is anybody over 16 moving with you? You must provide proof of their identity and address as listed in section one.

Are your children moving with you? You need to provide a copy of the child benefit or tax credits letter and this must have the child's name on it. Please note we do not accept bank statements as proof of child benefit.

We can only include children on your application if you provide this proof.

Section 3 - Local Connection

If you don't live in St Helens at the moment and are looking to move into the area, you can apply but will be given less priority than somebody who already lives in St Helens. If you can prove that you have a local connection to the St Helens area then we will be able to consider your circumstances further.

You will need to provide proof that you have:

  • Lived in St Helens for at least 2 of the last 5 years.
  • You have had settled employment in St Helens for more than 6 months.
  • You have close family relatives who currently live in St Helens.
  • You have special circumstances, for example: you provide support and care to someone in St Helens.

Section 6 - Why do you want or need to move?

Any information that you can provide to support this section will help us to process your application more accurately and quickly. Information you may want to include are copies of letters from any support agencies or professionals, information that proves you are ready to move on from supported housing, details of someone that you provide care for, any information that proves relationship breakdown, or an Environmental Health report if your situation includes poor living conditions.

Section 7 - Where you used to live

It is very important that you provide ALL of your previous addresses for the last 6 years. You must also provide details of your landlord's names and addresses as we may need them to provide a reference. We won't be able to process your application until you provide this information.

NB: If you have previously held a tenancy we may need you to provide a landlords reference. If you are unable to obtain a landlords reference we will ask you for further information.
